Where Is My Proof of Purchase

I recently purchased the Writing Mental Ray Shaders book by Andy Kopra. I have installed the Standalone Mental Ray that comes with the book, but when it asks me to input the Proof of Purchase I don't know where to find it inside of the book.

My question is where can I find the proof of purchase that comes with my book purchase. I also have the receipt and there is a number on the receipt which i tried using but that's also not the correct number.

I would appreciate any help with this ASAP, thanks.

Enclosed in the back flap, there should be a piece of paper named proof of purchase with a license code number on it. See the bottom of this page for an example.
